Your contact details, and the health information you choose to give us so we can assess you and plan your programme safely. If you sign up for our monthly letter, your email address.
Why we use it
To assess you, plan and run your programme, and contact you about your appointments. With your written consent, to update your own doctor. We never use photos, videos or stories of clients without separate written consent for each item, and you can withdraw that consent at any time.
Where it is kept
In our practice management system, kept confidential and seen by the HMG staff who need it, and no one else.
Your choices
You can ask to see or correct your data, withdraw a consent, or unsubscribe from our letter at any time.
